A useful care note says what you noticed and when. Keep the wording factual and avoid labeling a symptom with a diagnosis you have not received. The purpose is to communicate clearly with your veterinarian, not to replace their assessment.

Keep the clinic’s instructions accessible
Save official visit summaries, medication directions, and records in a folder for the correct pet. Record clinic contacts and confirmed follow-up appointments. If documents disagree or the instructions are unclear, ask the clinic to resolve the issue.
A calendar entry can point to the source document. It should not become a rewritten treatment plan based on your own interpretation.
Make shared notes understandable
Include the pet’s name, the date and time, the caregiver, and a concise description of the observation or completed task. If information is uncertain, say so rather than filling a gap from memory. Keep urgent communication separate from ordinary logging.
For prescribed tasks involving multiple people, use the medication record-keeping workflow and follow your veterinarian’s directions.
Prepare a focused appointment summary
Before an appointment, locate the relevant history and write down your questions. Bring the original documents the clinic requests. Avoid assuming an app note is an official vaccination certificate or a clinical report.
