For one dog or cat, put the pet’s name at the top and use one row per meal. For multiple pets, write the pet’s name in every row. Copy food and portion instructions from your existing care plan; this sheet does not recommend amounts or feeding times.

Enter the actual completion time and caregiver. A planned meal is not a completed meal. If you do not know whether a pet was fed, leave the uncertainty visible and check with the assigned person.

Make the log work for your household

Keep one current copy beside the supplies and agree who updates it. For a person away from home, agree how the latest entries will be shared. A blank row means “not recorded,” not proof that care did or did not happen.
